Singer Information Form

This electronic form must be submitted online along with the two paper forms, Emergency Medical Treatment Form and the COVID-19 Form--both paper forms described later in this electronic form. Only when an audition, this electronic form and the two paper forms have been printed, completed and submitted by U.S. Post or email will your 3rd - 12th grade singer's registration be complete for the 2025-2026 season. If you are applying for scholarship support, you will also need to complete the online scholarship packet. All scholarship support documents and/or letters of recommendation must be submitted no later than two weeks after your singer begins rehearsals. During the scholarship application period, your singer's tuition account will be placed on hold.

    This form should be completed by a parent or guardian and not by a singer. There are acknowledgements contained on this form that must be completed by an adult parent or guardian and should not be completed by a singer even if the singer is 18 years of age.

  • Payment Information

    LYC Annual Tuition is $650. Every enrolled singer will receive a quarterly tuition bill until their account is paid in full. You may choose to pay your account as you wish, so long as you make the minimum benchmark payments of $162.50 by each concert date. This means you can pay your account in full at the start of the season (preferred), pay monthly, or pay twice per season, once at the beginning and once in January. If your account is not current by each concert date, your singer will not be permitted to perform. Scholarships are available. And tuition rates can be prorated pending your singer's enrollment date/ scholarship award. By selecting "Yes, I agree" above, you understand the tuition amount, billing process and that scholarships are available based on need.

    You must complete an Emergency Medical Treatment Form (EMT) for each singer enrolled in Louisville Youth Choir. This form must be submitted every year, no exceptions, and singers WILL NOT be permitted to participate without this form on file.

  • COVID-19 Information

This form must be submitted every year, no exceptions, and singers WILL NOT be permitted to participate without this form on file.
