Primary Singer Enrollment Form

This form must be submitted online. Additionally the Emergency Medical Treatment Form and COVID-19 Form found on the LYC Website (www.LouYouthChoir.org) under LYC Families: 2025-2026 Enrollment Checklist, must be submitted via US Mail to LYC, 3105 Lexington Road, Louisville, KY 40206 or electronically to info@louyouthchoir.org. If you are applying for scholarship support, you will also need to complete the online scholarship packet. All forms must be submitted no later than one week after your singer begins rehearsals. After that time, placement for your singer will be temporarily suspended until form submission is complete.

  • Payment Information

    Primary Choir tuition is $175 per session. By clicking here, you are agreeing to pay your singer's tuition by cash, check or the online store with credit card at www.louyouthchoir.org. (Financial aid is available. Please email info@louyouthchoir.org for more information. LYC never turns away a singer due to the inability to pay for services.)

  • Medical Information

    You must complete an Emergency Medical Form for each singer enrolled in Louisville Youth Choir. This form is located on the LYC website at www.louyouthchoir.org. (Click LYC Familes: Enrollment Checklist). This form MUST be completed every year, no exceptions, and singers WILL NOT be permitted to participate without this form on file.
    EMT forms are located on the LYC website at www.louyouthchoir.org/majorchords - click FORMS - and must be completed, printed, signed and returned to LYC.

  • COVID-19 Form

    You must complete the COVID-19 Form for each singer enrolled in Louisville Youth Choir. This form is available on the LYC website - www.louyouthchoir.org - Click LYC Families: Enrollment Checklist. This form must be submitted every year, no exceptions, and singers WILL NOT be permitted to participate without this form on file.
    COVID-19 forms are located on the LYC website and must be completed, printed, signed and returned to LYC. LYC will accept these completed and signed forms as hard copies via U.S. post to 3105 Lexington Rd. Louisville, KY 40206 or as electronic copies at info@louyouthchoir.org.

  • Singer Handbook
